Generic Spiriva Handihaler (Tiova Rotacaps) is a long-acting, 24 hour, anticholinergic bronchodilator which is used for the long-term, once-daily, maintenance treatment of bronchospasm associated with chronic obstructive pulmonary disease(COPD) which includes bronchitis and emphysema.

Generic Spiriva Handihaler works by relaxing the muscles around the airways so that they open up and enables patients to breathe more easily.

Uses of Generic Spiriva (Tiova Rotacaps by Cipla)

Tiova Rotacaps by Cipla is to be used one time every day to control symptoms of chronic obstructive pulmonary disease (COPD). Tiova Rotacaps by Cipla (Tiotropium Bromide Inhalation powder capsules 18 mcg) helps make your lungs work better for 24 hours. Generic Spiriva Handihaler relaxes your airways and helps keep them open. You may start to feel like it is easier to breathe on the first day, but it may take longer for you to feel the full effects of the medicine. It works best and may help make it easier to breathe when you use it every day.

Tiova Rotacaps by Cipla (Tiotropium Bromide inhalation powder 18 mcg) also reduces the likelihood of flare-ups and worsening of COPD symptoms (COPD exacerbations). A COPD exacerbation is defined as an increase or new onset of more than one COPD symptom such as cough, mucus, shortness of breath, and wheezing that requires medicine beyond your rescue medicine.

COPD is a serious lung disease that includes chronic bronchitis, emphysema, or both. Most COPD is caused by smoking. When you have COPD, your airways become narrow and air moves out of your lungs more slowly. This makes it hard to breathe.

This is not a rescue medicine and should not be used for treating sudden breathing problems. Your doctor may give you other medicine to use for sudden breathing problems.

Tiova Rotacaps (Generic Spiriva Handihaler) – Dosage

The recommended dosage of Tiova Rotacaps (Tiotropium Bromide inhalation powder) is one Rotacap inhaled once daily, using the Cipla Rotahaler/ Revolizer.

Dosage adjustment is not required for geriatric, hepatically-impaired, or renally-impaired patients. However, patients with moderate to severe renal impairment, who are being treated with Tiova rotacaps, should be monitored closely. Tiova rotacaps must be used for inhalation only with the rotahaler device and must not be swallowed.

How to Use Tiova Rotacaps (Generic Spiriva Handihaler)

Use Tiova Rotacaps by Cipla (Generic Spiriva, Tiotropium Bromide inhalation powder 18mcg) once daily exactly as instructed by your healthcare provider. Tiova Rotacaps by Cipla (Generic Spiriva HandiHaler, Tiotropium 18mcg) should only be taken with the Rotahaler device through your mouth (oral inhalation). Read the leaflet supplied with the RotaHaler device and follow these instructions:

• Do not swallow the capsules
• Use the RotaHaler to take the medicine
• Do not use the RotaHaler device to take any other medicine
• The medicine comes as a powder in a capsule that fits into the RotaHaler device. Each capsule contains only a small amount of powder equal to one full dose of medicine.
• Take one of the capsules from the packaging and insert into the Rotahaler. Twist the RotaHaler to pierce open the capsule.
• After the capsule is pierced, take a complete dose of the medicine by breathing in the powder by mouth two times, using the RotaHaler device.
• Throw away any capsules that are not used right away after it is taken out of the package. Do not leave the capsules open to air; they may not work as well.

The RotaHaler comes with a pictogram of the above procedure making it very easy to follow and understand.

If you miss a dose, take it as soon as you remember. Do not take this medicine more than one time every 24 hours. If you use more than your prescribed dose of Tiova Rotacaps by Cipla (Generic Spiriva, Tiotropium 18mcg), call your doctor or a poison control center.

Generic Spiriva Inhaler – Contraindications

Generic Spiriva Handihaler (Tiova Rotacaps) is contraindicated in patients with a hypersensitivity to to Tiotropium, Ipratropium, or any inactive component of Tiova Rotacaps.

Tiova Rotacaps Side effects

The common possible side effects reported in patients treated with Generic Spiriva Handihaler (Tiova Rotacaps) include pharyngitis, cough, dry mouth and/or throat irritation and sinusitis .
Tiova Rotacaps by Cipla (Tiotropium Bromide inhalation powder) can cause serious side effects. If you get any of the following side effects, stop taking the medicine and get medical help right away:

• Allergic reaction including: itching, rash, swelling of the lips, tongue, or throat (trouble swallowing)
• Sudden narrowing and blockage of the airways into the lungs (bronchospasm). Your breathing suddenly gets worse.
• New or worsened increased pressure in the eyes (acute narrow-angle glaucoma). Symptoms of acute narrow-angle glaucoma may include: eye pain, blurred vision, seeing halos (visual halos) or colored images along with red eyes.
• New or worsened urinary retention. Symptoms of blockage in your bladder and/or enlarged prostate may include: difficulty passing urine, painful urination.

Other side effects of Tiova Rotacaps by Cipla (Generic Spiriva HandiHaler) include:

• upper respiratory tract infection
• dry mouth
• sinus infection
• sore throat
• non-specific chest pain
• urinary tract infection
• indigestion
• runny nose
• constipation
• increased heart rate
• blurred vision

This is not a list of all the possible side effects of Tiova rotacaps (Tiotropium Bromide inhalation powder). Tell your doctor if you have any side effect that bothers you or that does not go away.

Warnings & Precautions When Using Generic Spiriva HandiHaler

ONLY FOR INHALATION. Do NOT Swallow the Generic Spiriva capsule – it is dangerous. Generic Spiriva should only to be taken through oral inhalation with the Rotahaler device.
Do not use Tiova Rotacaps by Cipla (Generic Spiriva Handihaler) if you are allergic to Tiotropium Bromide or have had an allergic reaction to Ipratropium (Atrovent). Allergic reactions may include itching, rash, or swelling of the lips, tongue, or throat (trouble swallowing).

Before taking Tiova Rotacaps by Cipla (Tiotropium Bromide inhalation powder 18 mcg), tell your doctor about all your medical conditions, including if you:

• have kidney problems
• have glaucoma
• have enlarged prostate, problems passing urine, or a blockage in your bladder
• have a severe allergy to milk proteins.

Tell your doctor about all the medicines you take, including prescription and non-prescription medicines and eye drops, vitamins, and herbal supplements. Some of your other medicines or supplements may affect the way this medicine works.

This is an anticholinergic medicine. You should not take other anticholinergic medicines while using this medicine, including ipratropium. Ask your doctor  if you are not sure if one of your medicines is an anticholinergic.

Know the medicines you take. Keep a list of your medicines with you to show your doctor and pharmacist when you get a new medicine.

Do not let the powder from the capsule get into your eyes. Your vision may get blurry and the pupil in your eye may get larger (dilate). If this happens, call your doctor.

Tiova Rotacaps Over Dosage

High doses of Tiotropium Bromide inhalation powder (Tiova Rotacaps) may lead to anticholinergic signs and symptoms. However, there were no systemic anticholinergic adverse effects following a single inhaled dose of up to 340 mcg of Tiotropium Bromide in healthy volunteers. Additionally, no relevant side effects, beyond dry mouth, were observed following a 7-day dosing of up to 170 mcg of Tiotropium Bromide in healthy volunteers. In a multiple-dose study in COPD patients with a maximum daily dose of 43 mcg of Tiotropium Bromide over 4 weeks, no significant side effects have been observed.

Tiova Rotacaps (Tiotropium Bromide Inhalation Powder) During Pregnancy and Breastfeeding

The US FDA has classified Tiotropium Bromide (Tiova Rotacaps) as Pregnancy Category C. It is not known if Tiova Rotacaps by Cipla (Generic Spiriva Handihaler) could harm your unborn baby or if the medicine passes into breast milk.

There are no adequate and well-controlled studies conducted in pregnant women. Tiova Rotacaps should be administered during pregnancy only if the potential benefit justifies the potential risk to the fetes.

Clinical data from nursing mothers exposed to Tiotropium Bromide inhalation powder (Tiova Rotacaps) are not available. Based on studies in lactating animals, Tiotropium is excreted into breast milk. It is not known whether Tiotropium is excreted into human milk, but since many medicines are excreted into human milk and given the findings in rats, caution should be exercised if tiotropium bromide is administered to a nursing mother. You and your doctor will decide if Tiova Rotacaps is right for you while breastfeeding your baby.
